Heart Health (Led by the PCN regional RNs and RDs)

The Class is a single session intended for individuals and their family members who would benefit from learning about Basic cardiac health information.

Topics Covered include:

Basic pathophysiology, prevention, treatments, lifestyle and diet influences and SMART goal planning.

Review and instruction on Blood pressure monitoring.

Individuals can book follow up one on one with the RD as needed.
