DESCRIPTION:Diabetes Basics (facilitated by the PCN regional RNs and RDs) \nThis class is a single session is intended for individuals and family members who have recently had pre diabetic A1C and / or FBG flagged by their health care professional OR have recently been diagnosed with type 2 diabetes. \nTopics Covered include: \nbasic pathophysiology\, prevention\, treatments\, lifestyle and diet influences and SMART goal planning \nIndividuals can book follow up one on one with the RD as needed. \n*This session is intended for stable clients who can initiate diet and exercise changes to help manage their diabetes. \n*This is not for individual diabetes management. The facilitators do not manage client’s medication or insulin. \nNote: The Diabetes Education Centre (DEC) will continue to provide education and support for those who require education beyond the Diabetes Basics program.
